This form, upon acceptance, will add or remove your telephone number from the CodeRED Emergency Notification System. This system allows you to be contacted by telephone with a brief description of selected local emergencies and instructions on any action to take if required. If you have a landline telephone number in the city of Sandwich, you need not re-register that number. You may register your cell or other phone number using this form. If you prefer not to participate in the CodeRED system at all, you may also 'opt-out' using the appropriate check box.
